How do I register for these programs?

1. Read about our formation programs:

• Faith Formation program and schedule for grades K-5

• Sacramental preparation

2. Register online!

3. Pay the registration fee online (also below).

$90 per family for the program through July 1.
After July 1, the fee will increase to $100.
(Both registration and payment are required by July 1 to qualify for the $90 rate.)

4. Order the Family of Faith Volume III (green) books online at Sophia Press: English Edition or Spanish Edition. When you checkout, enter the discount code MATNH for a 45% discount off the listed price! You can also call in your order by calling Sophia Press directly at 800.888.9344. Order your books as soon as possible to ensure that they will arrive in time!

5. Order a Parent’s Guide and however many Children’s Activity Books that you would like to have.

For those not baptized at St Matthew Church:

BAPTISMAL CERTIFICATES REQUIRED: *To complete the registration, the office must SEE a Baptismal Certificate with the Parish SEAL for anyone who was not baptized at St Matthew. The office CANNOT accept a COPY of the certificate. Certificates are easily acquired from the parish of baptism by calling their office. If you think you have already submitted a baptismal certificate, please call our office to verify.
